Ladies and Gentlemen,
I need your help and support.
As you may or may not know, I have been actively training for the Ironman Triathlon. This event takes place in November, now a little more than six months away. I have devoted time since November of 2006 towards both physical training and efforts to raise money for three major charities that do good work to improve the lives of people that compose the majority of our patients and their families. These three charities are the American Heart Association, the American Cancer Society, and the American Diabetes Association.
So far we have received a good amount of contributions towards these causes. The goal to raise $1 million, however, is still far off, and so I need you all to open your checkbooks and send checks to me designated to one or all of these charities. Any level of contribution is appreciated, but please realize the importance of this effort: this is being done in order to raise awareness of what Family Medicine is about, and to support organizations whose focus are diseases that affect the vast majority of the population for whom Family Medicine is the prime caregiver.
Please send checks made out to one or all of those charities to me at:

We are keeping a detailed list of the donors and contributors, and you will be duly recognized and credited. For more information, you may call or email my friend and former office manager, Laurel Cohen. Messages can be left for Laurel at the office, 203-322-7070, and she will get back to you. I am very appreciative of the support and assistance Laurel has been providing in helping me to coordinate this endeavor. I’d also like to mention and thank my wife and family for their support.
Thank you once again for your time and hopefully your support and financial contributions.
Sincerely,
Alan T. Falkoff, M.D., D.A.B.F.M., F.A.A.F.P.
